Annuity Application Submission Tips
  1. All agents must be contracted with the appropriate carrier. This includes AML training and any exam that the carrier may require in order to sell annuities.
  2. If not appointed with carrier (preferably, contracting forms would be sent in prior to the application being submitted), include appointment paperwork with the annuity application if the state you are writing in is not a pre-appointment state.
  3. Submit an original completed application, with all required forms attached.
  4. Include the suitability and product disclosure forms.
  5. Some carriers require a replacement form in certain states-even if there is no replacement.
  6. Attach a check, transfer or 1035 exchange paperwork or a memo stating that funds will be sent.
